It seems the cause of the leak was the packer.

Actually, I don't think that ever happens, lol (based on my experience).

However, it seems he missed it this time.

 

The reason was a mistake that tends to happen when using a packer.

I'm sure you know that you can't inject grout unless you remove the entire outer casing.

Therefore, we’ll perform the injection after removing all the casings.

The site where the casing was removed was an all-packer job, and the grouting took place after all the casing had been removed, but,

Apparently, they didn't do it right away but administered the entire dose at the end of the day.

 

This is the reason.

That’s a mistake packers tend to make more often than you’d think. (The rule of thumb is to use just one anchor! lol)

Depending on the soil type, in particular, there’s a high likelihood that it would weather away quickly; apparently, at that time, it was rock-hard mudstone.

Claystone is seriously dangerous!

Even with extremely hard mudstone, once you’ve drilled a hole in it, weathering begins right there!
